this is a survey. do you normally provide a foundation wall at overhead doors, or maybe just thickened slab? this is for a 24' wide door into garage of multi-story condo. exterior pavement has steep slope down into garage, and will likely have trench drain there. building is supported on deep foundations.
go below frost depth for any part of the bldg you wish to stay at original grade.
if you can find a way to prevent frost from forming under the thickened slab, you could go that route. you would need to heat the garage, insulate the outside edge of the thickened slab, and extend the insulation horizontally below the driveway pavement.

i agree with the above statements as well...
when designing in canada i typically used foudation wall if the rest of the structure was supported by foundation walls. i used sm load bearing insulation under (and extending out beyond) thickened slab if it was a structure with an unheated garage and slab on grade.
in new zealand i always use slab thickening. we don't get frost heave here in canterbury, or anywhere i know of in the country. possible exception would be for the very bottom of the south island (where i have not yet done any design work).
