does anyone know where this document could be found? just looking for an old copy/download, etc.

that's a tough one. i didn't find it on biblio.com. i would post the request in the boiler and pressure vessel forum under mechanical engineering. we used to use that code a lot when i started out in the utility boiler industry. i'll bet someone has on old copy lying around.
